Ordering the ADA bundle is easy. Follow these steps:

2. Fill in the required fields, including the primary owner’s name (and secondary owner’s if required).
3. Enter the service dog’s breed and name. You can also upload a close-up photo taken at eye level for personalization.
4. Once you submit your information you’ll have the option to add accessories at a discount. After payment you will receive a link to your digital ID and access to your account.

We do not verify the doctor’s letter. You are responsible for ensuring you have the proper documentation. After registration you can upload your doctor’s letter to your account so it appears on your verification page.
